- Three Required Pre-Requisite Courses: Anatomy and Physiology I and II, Introduction to Psychology
- Concurrent Courses: English I, Intro to Ethics, Lifespan Growth and Development

- TEAS Test Results: The final score is to be uploaded with application
- Course in Progress Form (s): If you are enrolled in one of the required courses during application cycle, please print this form from the website and have current instructor fill it out and upload with application

Two (2) References: These forms are printable and are confidential (applicant will not see). References need to be sealed in an envelope with the lip of it signed by the requested person filling it out. They can be hand delivered or mailed to nursing department (address on website)
